This printable is designed to have a range of different mental health resources and family/friends so that when one feels harmful to themselves, they can contact support; whether that be a friend, organisation or a hotline.
There is space to list the following:
- Three personal contacts (e.g. family member), with two phone numbers
- Emergency contact (e.g. 911)
- Poison contact number
- Nearest hospital/emergency contact number
- Doctor/medical practice contact number
There is a location to provide the information of two different support organisations, where both a contact number and a website can be listed.
